SORRY, THE DEADLINE TO PREREGISTER HAS PASSED

Use this form to register to attend the 1998 TMS Annual Meeting February 14-18, 1998, in San Antonio, Texas. The deadline for preregistration is November 15, 1997, to qualify for the "early bird" rate or January 16, 1998, to qualify for the advance registration rate. Subsequent to January 16, it will be necessary to register personally at the meeting (at a slightly higher cost). Registrations received after January 16, will be processed using on-site fees. Students must send via mail or fax a copy of their school identification card to receive the student rates.

Refund requests must be in writing directly to TMS and postmarked no later than January 16, 1998. A $30 processing fee will be charged for all cancellations. No refunds will be issued after January 16.
